JOB SUMMARY/ESSENTIAL JOB FUNCTIONS:

Under the supervision of the Advanced Practice Registered Nurse, this position will participate in clinical care related to a Health Resources and Services Administration (HRSA) grant. This position will assist with delivery of care at mobile clinic sites, primarily in Lake and Lauderdale counties, including but not limited to, recording medical histories, assisting with SDoH assessments, and performing preliminary assessments and physical test.



DUTIES AND RESPONSIBILITIES:

Conducts patient health history assessment, obtains vitals, monitors patients for change in status, communicates findings to clinician, contributes and implements plan of care, and provides patient education.
Coordinates patient care, schedules patient appointments, triages patient phone calls, schedules referrals, and administers tests under clinician orders.
Drives the mobile health unit to the site of service, maintains medical supplies and inventories, refuels as needed, and empties gray water tank.
Enters data into electronic medical record (EMR) system and assists with coding and billing.
Performs finger sticks and draws blood for laboratory specimens according to protocol.
Assists patients with understanding treatment goals, identifies any barriers to treatment, and seeks to resolve any barriers to care.
Conducts daily pre-trip and post trip inspections; completes daily operational logs.
Assists research team with obtaining data for grant reporting.
Performs other duties as assigned.


MINIMUM REQUIREMENTS:



EDUCATION: High School Diploma or GED OR successful completion of Vocational LPN program. (TRANSCRIPT REQUIRED)



EXPERIENCE: Two (2) years of clinical experience (MA) OR two (2) year clinical experience (LPN). Must have valid driver’s license.



LICENSE/CERTIFICATION: CMA or LPN. Must be Certified or Registered in the State of Tennessee (Medical Assistant) or Licensed in Tennessee (LPN). maintain BLS for Healthcare Provider CPR or CPR/AED for Professional Rescuer certification. (CERTIFICATION REQUIRED)

Knowledge of medical terminology and medical vocabulary.
Strong written and verbal communication skills.
Ability to communicate with a diverse population.
Ability to accurately maintain an EMR system.
Ability to effectively prioritize and manage time.
Knowledge of tele-medicine.
