Job Description
Summary
The Community Health Worker (CHW) will be working in the Division of Pediatrics-Public Health on a 1- year research grant to test the effectiveness of a tool called the Family CARE Portfolio. The Family CARE Portfolio is an organizational, executive functioning tool designed to help families: Coordinate care, Advocate for themselves, Record important information & feel Empowered to take charge of their health and well-being. This study will be evaluating it's effectiveness among CHWS with pregnant/postpartum populations. The CHW will recruit participants from a clinic and assist them with care coordination and accessing community resources.
The standard work schedule is Monday - Friday, 8:00am - 5:00pm, but hours may vary.
Job Duties
- 70% - Direct Service:
- Helpsparticipants connect with resources in their area and assists with applications for eligible programs.
- Documentsall activities, service plans, and results.
- Maintains confidentiality per HIPAA guidelines in regards to patient information.
- 15% - Recruitment & Consent:
- Recruitsfamilies from target clinic.
- Explainsthe study and assistsfamilies during the consenting process.
- 10% - Research Activities:
- Collectsresearch data and entersthe data into the research databases.
- Attends research meetings.
- 5% - Community Outreach:
- Developsrelationships with community organizations and facilitates a warm hand off to families.
Minimum Qualifications
- High Schooldiploma or GED.
- Two years of relevantexperience in health/social services.
Preferred Qualifications
- Bachelor's degree in Social Work.
- Community Health Worker Certification.
- Bilingual (Spanish/English).
